Do you have a child with severe mental/behavioral health problems and don’t know where to turn? Are you ready to have fun as a family and feel supported by them? Then Hand-in-Hand may be just what you are looking for!
Hand-in-Hand engages family, friends, teachers, and mentors as a support for children, ages birth through six, with mental illness and/or severe behavior problems. Families who have children with emotional and behavioral issues will receive:
- Guidance through medical systems, social services and school systems
- Mentoring
- Assistance with referrals to community programs and services
- Support from families who have had similar experiences
- Networking activities, including family support groups
The Wraparound Process
Hand-in-Hand uses the Wraparound approach. Wraparound is a comprehensive case management service that “wraps” the entire family in care. Working with a wraparound facilitator, the family identifies their strengths and needs, then establishes goals, and begins to learn about resources with their community.
Eligibility
All children from birth through age 6 who live in Tarrant County (excluding the City of Fort Worth) and either have a mental health diagnosis or are at imminent risk of a diagnosis are eligible for wraparound services provided by Hand in Hand.
